Adult Application: This is the application that must be completed all registered adult leaders in the Crew.  The form must be completely filled out and turned into the Crew Advisor.  It is then signed by the Crew Committee Chairperson and Charted Organization Representative.  The application is then provided to the Denver Area Council for processing.

Annual Health and Medical Form: The State of Colorado requires the Health and Medical Form listed below.  However, when attending out of state camps, including High Adventure Bases,  the following health and medical form is to be used.

Colorado BSA Health and Medical Form:This form is valid for 24 months for persons under 40 years of age and 12 months for persons 40 years of age and older.  Who needs a Class 1? Anyone attending Cub Scout Day Camps and any overnight activities less than 72 hours.  Who needs a Class 3? Anyone attending a high Adventure Base or Boy Scout Camp (longer than 72 hours).

Youth Application: This is the application that must be completed by all youth members of the Venturer Crew.  The form must be completely filled out, including being signed by both the parents/guardians AND by the youth.  The form should be turned into the Crew Advisor who will sign the application and turn it into Denver Area Council for processing.
